2010 Symposium Registration Form
You can register using the following online form, via phone, or email. We can only accept personal or business checks, bank draft, or cash at the door on the first day of the symposium. We do encourage you to mail your registration fee to our office as soon as you have registered. The number of rooms at the Mauan Lani may be limited and are on a first come first serve basis. Please make hotel reservations as soon as possible. If you a speaker, please contact our office before you make personal arrangements at the resort. We have special rates for conference attendees as well as speakers and guests. Register early; you will generally get the lowest possible rates! We will be glad to accommodate your special needs however possible.
The registration fees have increased for the first time since the very first symposium 12 years ago. Costs of running the symposium have increased many fold during the last decade. We believe the symposium provides an excellent forum for continued education, CME credits, and many other benefits to our members as well as other community health care professionals. It remains to be the longest running medical symposium in Hawaii and the most economical way to get CME credits.

EHI Member Registration Fee: $75.00
General Registration (non-EHI Members) Fee: $475.00
Non-Physician Allied Health Professional Discounted Fee: $275
Exhibitor (Registration Included): $1,000.00 per Table
Student Fee: $20 per day; or $45 for 3 days
